(1) Curriculum Proposals Program, Pre-Major Development Proposal Form - For new program or pre-major development, this form should be used by a team established to address enrollment management issues prior to sending a Credit Program Addition form to the Curriculum Committee. This team should include stakeholders with expertise in the program or pre-major area such as faculty, deans, assistant vice presidents, and other staff identified by the provosts, vice presidents, and others.The six (6) forms used to process curriculum changes are included as "input" documents. Those preparing a proposal can simply tab and insert information as appropriate.Checklists for each type of proposal are provided as tools for planning and review purposes. In addition, central data bases have been established in the form of file lists for editing and retrieval of proposals. Specific file names are:

Proposals can be completed on screen and saved to a central data base by clicking the SAVE button at the end of the proposal form. Proposals may be identified and retrieved for editing or historical reference. Completed proposals and appropriate supporting materials (syllabus, program outlines, etc.) can be emailed with attachments directly to Sue Maffei, Curriculum Committee Secretary or sent through college mail to Mail Code DTC-4. (2) Faculty Credentialing Information and process forms related to faculty credentialing are provided. The initial Credit Faculty Credentials Application can be completed on screen, printed out for required signatures, combined with transcripts and supporting materials and transferred to the appropriate campus credentials office.

base on the server by clicking the SAVE button at the end of the application form.
